Case Study 3: Detection of Silent Hypoglycemia Detection of silent hypoglycemic episodes and provide a safety net for overnight periods is one of the most valuable uses of CGM therapy. What we are looking at in the graph below is a a patient who on SMBG was showing elevated blood sugar every morning, so overnight basals and corrections kept being increased to be more and more aggressive. CGM use shows us that the morning elevations were rebounds from overnight asymptomatic hypoglycemic episodes, a potentially dangerous situation. Since this patient is on pump therapy we have the ability to reduce their overnight basal delivery without losing daytime insulin coverage.
We can also clearly determine a patient’s Duration of Insulin Action on CGM data, allowing us to reduce insulin stacking with more accurate insulin onboard calculations both in manual and electronic calculations. Case Study 5: Fine-tuning Meal Boluses CGM also allows a clear view of bolus performance. Here we can see in the trendgraph that the early meal doses may be too low, and that the nighttime snack dose is not sufficient. |
